Worried about one of my cats

One of our kitties is extremely thin even though she eats all the time. At first I thought it was worms but the medicine did nothing and I've checked the box when changing it see if there anything and haven't seen anything. Our other cat is fine. So I can't figure it out. We have a vet appointment but I was wondering if any one had any ideas what was going on. Granted she is like 13 or 14 yrs old but I wouldn't think it would matter as she is well taken care of. Can anyone help?

There are a few things that come to mind right away, diabetes or kidney failure. Both are quite serious and should definitely be ruled out. Good luck at the vet, keep us posted.
