Question potty-training

i have tried potty-training my puppy, Genevieve, but nothing's seeming to work out...i've tried taking her out constantly, i've tried telling her "no, outside" and putting her outside when she goes inside...now she holds it when she's not in her crate, but doesn't let you know she needs to go outside...she then eventually goes on the carpet...and when in her crate, she always goes on the blanket...however, she will hold it for a bit when she knows it's almost time to go outside for morning time until she's out in the yard...but she won't poop in the crate, luckily...but how do i get her to go OUTSIDE ONLY???

also, her stool is solid at first as she goes, but then it's mushy, sort of like a more solid diarreah...this doesn't seem normal...

These are tips for re-housetraining your dog.. I used them for my Yorkie since tiny dogs are prone to housebreaking incidents. Good luck!
